At Bluebell Wood Children's Hospice, we are committed to supporting young people and their families at every stage of their care journey. Thanks to a generous project grant from the Masonic Charitable Foundation, awarded in partnership with Hospice UK, we joined forces with Barnsley Hospice to bridge the gap for young people transitioning from children’s to adult hospice services.

Through our research, we identified that staff needed tailored skills and knowledge to provide developmentally appropriate care and to offer meaningful health promotion opportunities for young people. We also recognised that these opportunities needed to be actively promoted, as many young people have multiple long-term conditions and follow very different care pathways compared to adult hospice patients.

Before this project, there was no integrated transition pathway linking children’s and adult hospice services. Staff had minimal training in supporting young people during this stage, and there were no dedicated spaces to engage them effectively.

With the support of the grant, we were able to work closely with Barnsley Hospice to create special transition-focused days for families. Our team played a vital role in enabling early engagement with young people, helping them and their families feel supported and prepared for the next stage of care.

The funding also allowed Barnsley Hospice to establish a dedicated family room, enabling a truly person-centred approach to care. Importantly, it strengthened collaboration between Bluebell Wood and Barnsley Hospice, giving staff the opportunity to share expertise, upskill their knowledge and work together to meet the needs of young people more effectively.

Today, through this partnership, we are not only reaching more families, carers, and young people, but also building meaningful, lasting connections. The collaborative model we’ve developed has become a blueprint for transitions between children’s and adult hospices across the UK.



Since 2017, the Masonic Charitable Foundation, in partnership with Hospice UK, has distributed over £3 million in project and education grants to hospices across England and Wales, supporting projects like this that have lasting impact on families and communities.
